Which follow-up appointments are needed, with whom, and how soon?
Who books them — the hospital or us?
Are any tests or results still pending?
Warning signs
What symptoms mean we should call the doctor?
What symptoms mean we should go straight to the ER?
Who do we call after hours?
Help at home
Who is coming to the home (home health, physical therapy), and when?
What equipment is needed, and who arranges it?
Is transportation home and to appointments sorted?
Write the answers down in the moment — discharge conversations move fast, and “I'll remember that” rarely survives the drive home.
