Hospital Discharge Questions Checklist (Free Printable)

The questions to ask before a parent leaves the hospital — medications, follow-ups, warning signs, and what help is coming home.

Free to use — print it, or choose “Save as PDF” in the print dialog.

Diagnosis & recovery

What happened, and what should we expect during recovery?
Are there any activity, diet, or lifting restrictions?
What does a normal recovery look like week to week?

Medications

Which medications are new, which changed, and which should stop?
When and how is each one taken?
Can we get a printed, reconciled medication list before we leave?

Follow-up care

Which follow-up appointments are needed, with whom, and how soon?
Who books them — the hospital or us?
Are any tests or results still pending?

Warning signs

What symptoms mean we should call the doctor?
What symptoms mean we should go straight to the ER?
Who do we call after hours?

Help at home

Who is coming to the home (home health, physical therapy), and when?
What equipment is needed, and who arranges it?
Is transportation home and to appointments sorted?

Write the answers down in the moment — discharge conversations move fast, and “I'll remember that” rarely survives the drive home.
